Community Connections Launches Benefits Navigator Position With State Grant

Community Connections has launched a new Benefits Navigator position to help people with disabilities in Southwest Colorado access critical benefits, thanks to a grant from the Colorado Disability Funding Committee (CDFC).

The CDFC, part of the newly established Colorado Disability Opportunity Office within the Colorado Department of Labor and Employment, awarded Community Connections funding earlier this year to expand access to disability-related benefit programs. The grant is supported in part by proceeds from the sale of Colorado’s historic license plates.

The Benefits Navigator will assist community members in completing complex applications for Medicaid, Supplemental Security Income (SSI), and other programs such as food assistance and energy support. These services are available to anyone with a disability in the region, and is not limited to current Community Connections clients. Services will be offered in both English and Spanish.

“Applying for benefits is often overwhelming, and delays or denials can leave people without the services they need for months,” said Hannah Zorn, Community Connections Intake Case Manager and supervisor of the project. “This grant allows us to dedicate time and expertise to make the process smoother and ensure more people get connected to the supports they need to thrive.”

The Benefits Navigator role is being filled by Heleny Zacamolpa, a bilingual member of Community Connections’ case management team who has experience working with families in the Family Support and Early Intervention programs. Zacamolpa will provide direct assistance to individuals navigating the benefits system and guide them through applications and appeals.

The grant runs through June 30, 2026, with hopes of renewal based on demonstrated community impact.
