Community Asked to Participate in Public Health Survey

San Juan Basin Public Health (SJBPH) is pleased to announce the launch of the 2023 Community Health Assessment.

We value your input and ask that you find the time to take the survey yourself and to share it with others. Your health matters. Your voice matters…

SJBPH has launched this survey to gather input from La Plata and Archuleta county residents on their primary public and environmental health priorities and needs. The results of this community health assessment will guide a public health improvement plan that will inform the service and program priorities in both counties.

SJBPH’s survey is open through April 7 to any Archuleta or La Plata county resident and should take no more than 15 minutes to complete. All responses are anonymous.

SJBPH conducts community health assessments as part of a public health improvement plan every five years, in keeping with Colorado statutory requirements. Both Archuleta and La Plata counties are in the process of forming individual health departments to launch on January 1, 2024 following SJBPH’s dissolution. Input received from the community is essential and will be provided to both counties.

Public health relies on participation, and this is a great opportunity to share your input about health priorities and the state of community health in La Plata and Archuleta counties. The survey is easy to fill out and anonymous. Not only will the results inform public health programs and services in the years to come, but they are also shared with organizations and government agencies across the region to help guide their planning as well,” said Liane Jollon, SJBPH’s executive director.

In addition to the survey, SJBPH will hold informal focus groups and key informant interviews throughout the jurisdiction to discuss public health and wellness indicators with targeted populations.
