San Juan Basin Public Health (SJBPH) on April 7 will close its survey to gather input from La Plata and Archuleta county residents on their public and environmental health priorities and needs. The results of this community health assessment will guide a public health improvement plan that can inform the service and program priorities in both counties.
The survey opened in January, and has thus far drawn more than 1,200 responses across both counties. It is available in English and Spanish in electronic and paper formats.

SJBPH conducts community health assessments as part of a public health improvement plan every five years, in keeping with Colorado statutory requirements. Both Archuleta and La Plata counties are in the process of forming individual health departments to launch on January 1, 2024 following SJBPH’s dissolution. Input received from the community is essential and will be provided to both counties and community partners.
In addition to the survey, SJBPH is conducting informal focus groups and key informant interviews throughout the jurisdiction to discuss public health and wellness indicators with targeted populations.
The community health assessment survey takes approximately 15 minutes to complete. All responses are anonymous.
