Your health matters and so does your voice!
If you live in Archuleta or La Plata County, you are invited to complete the Community Health Assessment Survey being conducted by San Juan Basin Public Health.
SJBPH has launched this online survey to gather input from La Plata and Archuleta county residents on their primary public and environmental health priorities and needs. The results of this community health assessment will guide a public health improvement plan that will inform the service and program priorities in both counties.
SJBPH’s survey is open through April 7 to any Archuleta or La Plata county resident and should take no more than 15 minutes to complete.
Both Archuleta and La Plata counties are in the process of forming individual health departments to launch on January 1, 2024, so this community health assessment will be more important than ever. The input you provide will help the counties identify public and environmental health priorities that will inform their paths forward.
The survey will take approximately 15 minutes to complete, and all responses are anonymous. The data will be sorted by county, so please be sure to indicate your county of residence.

To receive a paper version of this survey, please call SJBPH at 970-247-5702 in La Plata County or 970-264-2409 in Archuleta County.
SJBPH conducts community health assessments as part of a public health improvement plan every five years, in keeping with Colorado statutory requirements.
“We urge residents to take the community health assessment survey and pass it along to friends and family. Public health relies on participation, and this is a great opportunity to share your input about health priorities and the state of community health in La Plata and Archuleta counties. The survey is easy to fill out and anonymous. Not only will the results inform public health programs and services in the years to come, but they are also shared with organizations and government agencies across the region to help guide their planning as well,” said Liane Jollon, SJBPH’s executive director.
In addition to the survey, SJBPH will hold informal focus groups and key informant interviews throughout the jurisdiction to discuss public health and wellness indicators with targeted populations.
