Do you have employees who still need to be vaccinated? Are they having trouble finding a vaccine clinic or making time to get vaccinated?
The Workplace Vaccination Program is a great opportunity provided by the State of Colorado to host a vaccine event at your workplace.
You will provide the date, time, and location (your workplace), and the State and its partners will provide vaccine and coordinate the planning and administration of the vaccine on-site.
This is an easy process, please fill out the following form (application) and let San Juan Basin Public Health know that your business has applied. Letting SJBPH know will help us track and follow-up on your application.
