Community Connections is proud to announce that Hannah Zorn, Intake Case Manager, has been honored as Case Manager of the Year by Alliance Colorado, a statewide association representing organizations that support people with intellectual and developmental disabilities. This prestigious award recognizes Hannah’s exceptional commitment to empowering individuals and families navigating long-term services and supports (LTSS) in Southwest Colorado.
“Hannah was nominated for numerous reasons,” said Elizabeth Fabrey, Vice President of Case Management at Community Connections. “Her deep knowledge of our programs, coupled with her compassion and integrity, ensures that every person and family she works with receives the highest level of support. She goes above and beyond to advocate for system-wide improvements, create educational resources, and remove barriers to care. She’s also an incredible teammate – always stepping up to support colleagues and the organization as a whole. We are truly lucky to have her.”
As the first point of contact for families beginning the LTSS process, Hannah plays a critical role in making a complex system more accessible and person-centered. Under her leadership, Community Connections has maintained a backlog-free intake process, even during major system-wide transitions. Recently, she developed an Intake 101 course to educate both internal and external stakeholders, ensuring that clients understand what to expect when applying for services.

Beyond her work with clients, Hannah is known for her proactive problem-solving and unwavering dedication to her team. When a colleague experiences a medical or family crisis, she is often the first to offer support. She volunteers her time, energy, and expertise without hesitation, embodying Community Connections’ core values of advocacy, empowerment, and trust.

As of July 2024, Community Connections has transitioned into the role of state-designated regional Case Management Agency, serving over 1,000 people annually across five counties in Southwest Colorado. The organization provides case management, early intervention, family support services, and care coordination to people with disabilities, developmental delays, and complex medical needs.
